Why Every Project Needs a Decision Log

Three months after a project closes, the most expensive question becomes 'why did we choose X?'. A decision log answers it in 30 seconds.

Decisions aren't free. The cost of forgetting them is paid quarters later.

Add a page inside every project called 'Decision log'. Every meaningful choice — vendor pick, architectural call, scope cut — goes in as one entry: date, the decision, the alternatives considered, the reason. Three lines max.
